Massage Therapist License
Overview
The City of New Berlin mandates that all Massage Establishments and Massage Therapists obtain proper licensing and adhere to specific regulations to promote public health and safety. To operate a massage establishment or practice as a massage therapist, individuals must comply with health and safety standards, submit a completed application form, and pay the required fees. The detailed requirements and regulations are outlined in the New Berlin Municipal Code and the Wisconsin state statutes.

Types of Massage Licenses
- Massage Establishment: $150 annually, licensees may preform massage services in specified areas listed on their application.
- Massage Therapist: This license is required for anyone performing massage services in a Massage Establishment. This license is issued at no cost upon verification of the applicant’s valid State License.
Massage Establishment applications
- To apply for a Massage Establishment License, complete all forms in the Massage Establishment Packet and ensure each individual listed in Part C fills out a Questionnaire (contact our office if additional copies are needed). The application will be reviewed by both the City of New Berlin Police Department and the Clerk’s Office, and both approvals are required before the license can be issued.

Massage Therapist application
- Massage therapists in New Berlin must be licensed and comply with city and state health regulations. Applicants need to show proof of having a State License and must submit an application with liability insurance.
